I consent to my child's participation in the Steubenville Conferences with CoMo LifeTeen from July 10-12, 2026 at Missouri State University in Springfield, MO. I release CoMo LifeTeen (Newman, Sacred Heart, Our Lady of Lourdes), Steubenville Conferences, and all staff and volunteers from liability with respect to injury, illness, robbery, or accident that may occur to my child during this activity. I understand that there will be light physical activities in which my child may be a participant. Therefore, I give my consent for treatment in case of injury, accident, or illness, and I waive any and all liability from any person treating my child. In the event emergency medical or surgical treatment is necessary, I give permission for conference staff to transport my child to a hospital either by private car or ambulance to receive necessary treatment. I agree to accept all financial responsibilities and relieve CoMo LifeTeen, the Steubenville staff, and volunteers of all responsibility and consequences that may arise from this treatment. If my child does not abide by the rules set forth or becomes ill, I will pick up my child within a reasonable time.
